 You will need the Adobe Reader to view and download the Schedule, Application, Liability Release Form and Medical Form. After registering your child online, all three forms must be completed for each child. You may also pick up a copy of these forms at the church. These forms must be submitted by June 23rd. You can fax the completed forms to 516-292-2821 or mail them to:
